See forum posts:

Flash graphs are not loading for some users, which might be related to the PDF plugin but I have no idea why this would be the case.

how would the .htaccess file affect the graph working or not?

If Apache isn't correctly configured, it'll see the .htaccess file and simply terminate the request (500 Internal Error).

A piwik user created a bug for Firefox https://bugzilla.mozilla.org/show_bug.cgi?id=614217 - so this is not a piwik bug
